PACKAGE 3: ADULT NDIS REVIEW ASSESSMENT

It is common for us at Brilliant Minds Psychology ro receive phone calls from NDIS Participants that hve been asked by their support coordinator or plan manager to have a "DSM-5" Assessment. We have created this package to support this request. This package provides a reivew of your current NDIS Plan, reviewing outcome goals and conducting further assessment. This package is. most suitable for those NDIS participants with a current diagnosis of Autism Spectrum Disorder or Intellectual Impairment and is already a recipient of NDIS Funding. Our assessment is disability friendly which m eans we go through all questionnaires with you in session. The NDIS Review Assessment Package is useful in evaluating an adults current situation and needs agains their NDIS goals, current plan and future treatment support plan. The NDIS Review Assessment Package includes the following: Session 1: Up to 2 hour intake session - including a review of your current goals in your NDIS plan, supports currently receiving, identifying ongoing goals. This session also focuses on obtaining background information. This interview further assesses your adaptive living skills using an evidence questionnaire. (up to 120 minutes). This first session will further assess whether this is the most appropriate package for you.Following this interview questionnaires are sent to a parent or carer for collateral information when possible and with client consent.Session 2 : Up 2 to hours - The second session focuses on assessment of social skills and disability related questions (60 to 120 minutes). All items used are evidence based measures. Session 3: 90-120 minutes. Cognitive Assessment - WAIS-IV (90-120 minutes) plus 1 hour scoring results.Session 4: 1 hour - feedback sessionComprehensive Report: A comprehensive report based on 4 hours of clinical time addressing NDIS Goals, Current Supports, NDIS Critieria and summarising all assessment results. (Report is completed within 2-3 weeks of assessment completion).
Please note we charge additional fees based on our hourly rate for any work beyond this scope.
We have our team of psychologists and provisional psychologists that conduct Package 3. All reports are signed off by our Director and Clinical Psychologist Dr. Jenni Silva.
This assessment is based on 12 hours of time.
