- 3 Hour Online Interview with Breaks (can be broken up over different days) utilising MIGDAS
- Comprehensive Report with Recommendations (based on 4 hours)
- OPTIONAL - Feedback Session (30 minutes - Optional) Additional $140 should you wish for this to be included

If a clinical psychologist sign-off is required for an NDIS application, an additional rate of $260 will apply upon request. If an NDIS request form is recommended to be completed by a therapy following assessment, additional fees will incur (approximately 1 hour)

For those seeking medication pathways for the treatment of ADHD, a psychiatrist assessment/appointment is needed. Our report can be shared with your psychiatrist to assist in this process. However it is important to note that all psychiatrists will follow their own assessment process and a diagnosis provided by a psychologist does not guarantee that a psychiatrist will prescribe medication and or may hold a different professional opinion.

About Sally Smith Clinical Psychologist Registrar
Sally is an empathetic and collaborative Psychologist with a passion for working wih adolescents and young adults. She places great importance on creating a non-judgemental, respectful and safe space. Sally has worked in a number of settings including private and public youth mental health, schools, forensic and not for profit community settings. She has worked as a Psychologist in both Australia and Germany. Sally provides neuroaffirming therapy and adult ADHD and Autism assesments.
Sally loves helping Neurodivergent people understand themselves better, celebrate their strengths, learn coping streategies and advocate for accomodations. Sally's areas of service are to adolescents 15 years and over and adults under 40.
Availability: Wednesday 12pm to 7pm and Friday 11:00am to 6:00pm
