Before completing the screener forms, we ask that you please read the below information:
Why am I required to complete a screener form?
This form will help us determine if we are a suitable service provider.
Once this form is completed, we will review the form. Depending on suitability and availability, we will give you a call to offer you appointments. Alternatively, if there is no suitable psychologist available, we will send you a waitlist confirmation email and contact you when spots become available.
Please note that we cannot book any appointments until this online form is completed. Brilliant Minds Psychology is a private clinic and is not a suitable option for those requiring a high level of support. If we are not a suitable service, we will contact you via email and recommend that you contact your General Practitioner (GP). When offered an initial appointment, please note that we will require the full payment/signed NDIS Service Agreement to secure the first session. This payment is non-refundable within 48 hours of the appointment time.

What happens when a spot becomes available?
When appointments become available, we will give you a call to arrange appointments. If we cannot get a hold of you via phone call, we will send you an email. Please note that we cannot hold appointments, so if you have a missed call from us, we ask that you call us as soon as possible. We recommend that you save our number (07) 3133 0610 on your phone as a contact so you know that we are calling.
Note: We kindly ask that you are flexible with your appointment day/time preference at the start (if at all possible with your schedule). If you require an after-school appointment, you may be waiting longer for this to become available due to the high demand for these times and limited number of spots available.
Kindly note that we aim to accomodate to your appointment day/time preferences as soon as we can!
What do I do if I need to see someone right now?
Unfortunately, we may not be able to see you as soon as you may be required. If you are seeking immediate assistance and/or in the event of an emergency, please get in touch with 000 immediately or head to the emergency department at your local hospital.
Alternatively, we recommend speaking with your GP to speak about alternate pathways in the meantime whilst you are awaiting services with us. Brilliant Minds Psychology operates as a private practice, and we do not provide immediate emergency services. If your child is in immediate danger or requires urgent assistance, we strongly recommend taking the following actions:
