About the Package

Child and Adolescent Functional Capacity Assessment (FCA)

The functional assessment is a complete assessment of how your child is functioning in their everyday activities at school and home environments. The Brilliant Minds Psychology functional assessment will assess fine motor skills (shoelaces, dressing, writing, manipulating objects) gross motor skills (running, balancing, jumping, gymnastic movements) and visual perception skills (hand-eye coordination, figureground discrimination). Your Occupational therapist will then use the information gathered through this assessment to provide intervention recommendations and assess adaptive functioning. This package is suitable for NDIS participants who are applying for NDIS funding or who are about to review their current NDIS funding. Assessments used in this package will vary depending on the age of the individual being assessed. Questionanires that are typically sent out as a part of this package include the Vinelands, WHODAS, BRIEF, SRS, Sensory Profile.
Results from this assessment cover all domains required for NDIS funding applications including Communication, emotional regulation and behaviour, cognition, self-care and self-management, learning, mobility and social interactions. This package is suitable for participants who have a current diagnosis, as this is not a diagnostic assessment. This package is suitable for children and teens. Our OT Functional Capacity Assessments (FCAs) are tailored to help you access the NDIS funding and supports that empower you to live independently and reach your goals.

The Assessment Breakdown/How It Works

Session 1: Parent/Guardian Intake Session
1 Hour Session Information gathering session. Includes a review of your current goals in your NDIS plan (or development of goals if you are applying for NDIS), supports currently receiving, and identifying ongoing goals. This can be conducted in clinic or via telehealth
Session 2: Child Session 1
1 Hour Session with Child and Parent Must be Conducted In Clinic Premises
Session 4: Parent/Guardian Feedback Session
1 Hour SessionIn Clinic or via Telehealth4 to 6 weeks from assessment date (depending on complexity)
Comprehensive Report
A comprehensive report based is provided including scoring and interpretation of the tools used, addressing NDIS Goals, CurrentSupports, NDIS Criteria and summarising all assessment results.
How Is The Assessment Scheduled?
We generally schedule the assessment to occur over the course of a single day. The feedback session will be arranged within a 4 to 6 week timeframe.

Cost

From $1980

This assessment is based on 9 to 15 hours of clinical time. You will be provided with a confirmed total after the intake session. Please note we charge additional fees based on our hourly rate for any work beyond this scope. If a home or school visit is requested, additional charges and travel costs apply. A $500 non-refundable deposit or signed NDIS Service Agreement is required at the time of booking. This assessment is not covered by Medicare. If you have private health insurance, please contact your provider to see how much they can cover for you. This is not a diagnostic assessment. We accept NDIS Self and Plan Managed Clients.
Important Note About Teacher Questionnaires Brilliant Minds Psychology provides online questionnaires for your child's teacher as part of the package. However, some schools often require hard copy questionnaires. Here are your options:
1. Digital Questionnaires (Included in Package)We will send digital versions of the questionnaires for the teacher to complete. 2. Hard Copy Questionnaires (Additional Fee)If the school requires hard copies: An additional fee of $65 will apply. This includes express postage with tracking. 3. Self-Delivery of Hard Copies (Additional Printing Fee)If you prefer to pick up and deliver the questionnaires yourself: Cost is $55.
