Package 1: Adult Functional Capacity Assessment

The functional assessment is a complete assessment of how your child is functioning in their everyday activities at school and home environments. The Brilliant Minds Psychology functional assessment will assess fine motor skills (shoelaces, dressing, writing, manipulating objects) gross motor skills (running, balancing, jumping, gymnastic movements) and visual perception skills (hand-eye coordination, figureground discrimination. Your Occupational therapist will then use the information gathered through this assessment to provide intervention recommendations. Our Functional Capacity Assessment is unique in that it includes a cognitive screen using the TONI-4. This package is suitable for NDIS participants who are applying for NDIS funding or who are about to review their current NDIS funding. Results from this assessment cover all domains required for NDIS funding applications including Communication, emotional regulation and behaviour, cognition, self-care and self-management, learning, mobility and social interactions. This package is suitable for participants who have a current diagnosis, as this is not a diagnostic assessment. This package is suitable for children, teens and adults. Assessments used in this package will vary depending on the age of the individual being assessed. What Is Involved? Session 1: 1-hour intake session with an Occupational Therapist, and includes a review of your current goals in your NDIS plan (or development of goals if you are applying for NDIS), supports currently receiving, and identifying ongoing goals. This session also focuses on obtaining background information. Following this interview questionnaires are sent to the client, parent or carer for collateral information when possible and with client consent. Session 2: 1-hour session with an Occupational Therapist including additional assessment procedures. Session 3: 1 hour Cognitive Screen using the Test of Nonverbal Intelligence, Fourth Edition (TONI-4). This is a cognitive screen administered by your OT. This is recommended as part of your assessment if no cognitive assessment has been completed within the past 2 years. Session 4: 1-hour feedback session with an Occupational Therapist Comprehensive Report: A comprehensive report based is provided including scoring and interpretation of the TONI-4 addressing NDIS Goals, CurrentSupports, NDIS Criteria and summarising all assessment results. (Report is completed within 4-6 weeks of assessment completion). Please note we charge additional fees based on our hourly rate for any work beyond this scope. If a home visit is requested, additional charges and travel costs apply. This assessment cost is based on 11 - 15 hours of time
