Package 3: NDIS Review Assessment
It is common for us at Brilliant Minds Psychology to receive phone calls from NDIS participants that have been asked by their support coordinator or plan manager to have a "DSM-5-TR" assessment. We have created this package to support this request. The package provides a review of your current NDIS plan, reviewing outcome goals and conducting further assessment. This package is most suitable for those with a current diagnosis of Autism Spectrum Disorder (ASD) or Intellectual Disability and is provided with NDIS funding.
Our assessment package is disability friendly which means we go through all questionnaires with you in session. The NDIS Review Assessment Package is useful in evaluating an adults current situation and needs against their NDIS goals, current plan and future treatment support plan.
This package reviews intellectual ability, adaptive living skills, current level of need, and whether the current provided level of NDIS support assisted in reaching an individual's goals. The review allows for core support and capacity building recommendations, along with what future support may be required. Such identified information is mapped against the NDIS Allied Health review recommendations.
The NDIS Review Assessment Package includes the following:
Session 1: Intake Session (2 Hours) - including a review of the current goals in a NDIS plan, current supports and identification of ongoinggoals. This session focuses on obtaining background information and adaptive living skills.
Session 2: Assessment of Social Skills and Disability related questions using evidence based measures (2 Hours)
Session 3: Cognitive Assessment using WAIS-IV (2 Hours)
Session 4: Feedback Session (1 Hour)
A comprehensive report will be provided addressing NDIS goals, current supports, NDIS criteria and summarising all assessment results. Please note that there is a two to three week turn around for report writing. There will be a four week turnaround for report writing if the assessment is booked in with a Provisional Psychologist. You will be provided with this information at the time of booking an assessment.
The total cost of this assessment is based on 13 hours of time.Please note that a $500 deposit is required at the time of booking.
