About the Package

Adult Functional Capacity Assessment (FCA)

Our Functional Capacity Assessments (FCAs) are tailored to each individual. We take the time to understand your needs, ensuring a personalised assessment. Our Functional Capacity Assessment evaluates your capacity to engage in daily activities within the familiar settings of your residence, workplace, and recreational spaces. These assessments encompass an examination of your overall well-being, level of independence, support requirements, mobility, communication skills, self-management abilities, learning aptitude, and self-care capabilities while engaging in various tasks. Your Occupational Therapist will then use the information gathered through this assessment to provide intervention recommendations.
Our Functional Capacity Assessment is unique in that it includes a functional cognitive screen using the BADS (Behavioural Assessment of the Dysexecutive Syndrome). The BADS is specifically designed to evaluate cognitive functioning in a practical, real-world context, ensuring that our assessments are highly relevant to occupational therapy. This package is suitable for NDIS participants who are applying for NDIS funding or who are about to review their current NDIS funding. Results from this assessment cover all domains required for NDIS funding applications, including communication, emotional regulation and behaviour, cognition, self-care and self-management, learning, mobility, and social interactions. We provide questionnaires and scales, such as the Vineland or ABAS, offering deeper insights to support a comprehensive assessment. This package is suitable for participants who have a current diagnosis. This is not a diagnostic assessment.
Assessments used in this package will vary depending on the individual being assessed.

The Assessment Breakdown/How It Works

Session 1: Client Session
2 Hour Session with an Occupational Therapist (OT), and includes a review of your current goals in your NDIS plan (or development of goals if you are applying for NDIS), supports currently receiving, and identifying ongoing goals. This session also focuses on obtaining background information. Following this interview questionnaires are sent to the client, parent or carer for collateral information when possible and with client consent. In Clinic or via Telehealth
Session 2: Feedback Session
1 Hour SessionIn Clinic or via Telehealth4 to 8 weeks from assessment date (depending on complexity)
Comprehensive Report
A comprehensive report based is provided including scoring and interpretation of the BADS, addressing NDIS Goals, CurrentSupports, NDIS Criteria and summarising all assessment results.
How Is The Assessment Scheduled?
We generally schedule the assessment to occur over the course of a single day. The feedback session will be arranged within a 4 to 6 week timeframe.

Cost

From $1980

This assessment is based on 9 to 15 hours of clinical time. You will be provided with a confirmed total after the intake session. Please note we charge additional fees based on our hourly rate for any work beyond this scope. If a home or work visit is requested, additional charges and travel costs apply. A $500 non-refundable deposit or signed NDIS Service Agreement is required at the time of booking. This assessment is not covered by Medicare. If you have private health insurance, please contact your provider to see how much they can cover for you. This is not a diagnostic assessment. We accept NDIS Self and Plan Managed Clients. Private clients accepted.
