  • Please kindly note that there are costs associated with this request. Please complete the following form if you require any of the following:
  • NDIS Review Report
  • Letter for a Third Party (e.g., school, allied health professional)

Request for Change of Appointment Days/Times/Frequency

Please be aware that we may not be able to assist with your request immediately. By completing this form, you acknowledge that we will record your preference in our system. If we are able to accommodate your preference promptly, we will notify you accordingly. We sincerely appreciate your patience and understanding. Please be advised that there is currently an extended wait time for after-school appointments due to high demand and limited availability. Opportunities for these appointments are rare.
